Preserve source IDs, wording, configuration, and assumptions in the output; do not modify a connected system or assert approval. ## Inputs and boundary Collect the signed review record, action IDs and wording, owners, due milestones, closure criteria, submitted evidence, version and configuration, and decision authority. Preserve the original action text. If there is no agreed closure criterion, flag the action for clarification before judging it closed. ## Method For each action, identify the exact concern and required outcome. Match each submitted artifact to the outcome, check its approval/version, and test whether the evidence resolves the root issue rather than merely documenting activity. Distinguish “done by owner” from “accepted by reviewer.” Identify related actions, dependencies, and whether one change reopens another finding. For a changed design, inspect effect on requirements, verification, risk, and prior review assumptions. Mark statuses with plain meanings such as open, evidence submitted, needs rework, or accepted by authority; do not invent new formal project lifecycle states. Consolidate duplicate actions only after confirming they share both closure criteria and approval path. Escalate overdue or blocking actions according to the project’s own gates, without manufacturing schedule impact. ## Deliverable Deliver a matrix with action ID, original decision, owner, closure criterion, evidence ID/version, reviewer assessment, current status, blocker/dependency, and authority/date. Include unresolved evidence requests. ## Miniature example PDR action A-8 asks to “show independent power for channels.” A revised schematic adds separate regulators but retains one upstream converter. The action remains open unless the board clarifies that this common source is acceptable or another control addresses it. ## Acceptance checks - Every closure maps to the original concern. - Evidence and reviewer acceptance are separate. - Open dependencies remain visible. Missing inputs permit a bounded partial assessment with specific evidence requests, not invented values, failure rates, clauses, approvals, or safety decisions. Keep analysis rows in the delivered document or spreadsheet; they do not create new exchange Item Types or Relationship Types. If proposing model edits, retain the original records and route proposals through the project’s authorized validation and review process. A responsible engineer or authority owns formal acceptance.
