Preserve source IDs, wording, configuration, and assumptions in the output; do not modify a connected system or assert approval. ## Inputs and boundary Collect the architecture or parts/functions list, interfaces, modes of operation, mission phase, diagnostic behavior, maintenance concept, and existing hazard or requirement references. Set the analysis level: function, equipment, board, or part. Record version and boundaries before rows are created. ## Method For each analyzed element, identify specific failure modes (open, short, stuck value, drift, late output, loss of power, or software behavior where applicable) from its actual function. Trace the local effect through the next higher assembly to the end effect; distinguish a fault from the hazardous condition it may contribute to. Identify detection method, detection latency, isolation or recovery action, and latent exposure. Check interfaces, safe-state transition, common supplies, maintenance-induced failure, and degraded operating modes. Connect rows to hazards and requirements when the source supports the relationship. If severity categories are provided, use their defined end-effect criteria; do not invent an RPN, multiply ordinal scales, or treat ranking as risk acceptance. If failure-rate data are absent, state qualitative coverage only. ## Deliverable Deliver an FMEA table with element/configuration, function, failure mode, cause where known, local and end effects, operating mode, detection and recovery, existing design control, source, unresolved question, and action owner. Keep causes distinct from modes and mark speculative causes. ## Miniature example For a pressure sensor stuck at a plausible value, the local effect is stale measurement; the system effect depends on voting and annunciation. If neither is documented, the row identifies a possible misleading indication and asks for voter logic; it does not assert a catastrophic end effect. ## Acceptance checks - Failure modes align with real element functions. - Every row has a traced end effect or an explicit unresolved end effect with the missing mechanism identified; never leave that column implicit. - Detection claims identify a mechanism and evidence. Missing inputs permit a bounded partial assessment with specific evidence requests, not invented values, failure rates, clauses, approvals, or safety decisions. Keep analysis rows in the delivered document or spreadsheet; they do not create new exchange Item Types or Relationship Types. If proposing model edits, retain the original records and route proposals through the project’s authorized validation and review process. A responsible engineer or authority owns formal acceptance.
