Preserve source IDs, wording, configuration, and assumptions in the output; do not modify a connected system or assert approval. ## Inputs and boundary Collect the aircraft or system function list, intended operations and phases, interfaces, crew or operator response assumptions, environmental conditions, and project-approved severity definitions. The FHA is function-centered; component implementation is supporting context, not a substitute for examining what the function can do wrong. ## Method For each function and phase, examine loss, erroneous output, misleading information, untimely operation, inadvertent activation, and degraded performance. Write the failure condition from the user or aircraft effect outward, then examine compensation, annunciation, exposure duration, and combinations with other conditions. Separate consequence assumptions from evidence. Where applicable, distinguish aircraft-level and system-level effects; do not silently transfer a system classification to an item. Reconcile duplicate failure conditions and check that architecture choices have not hidden functions. Identify safety objectives or follow-on analyses, but do not turn a severity label into an FDAL, IDAL, software level, or hardware DAL. Use the controlled ARP4761A/ED-135 or agreed method only after confirming the program basis and edition. Escalate disputed operational effects to the responsible safety authority. ## Deliverable Deliver an FHA table: function, phase, failure condition, operational effect, compensating action and assumptions, severity proposal if supported, rationale, source, follow-on safety objective or analysis, and unresolved reviewer decision. Preserve the function wording and source revision. ## Miniature example For “display cabin pressure,” a blank display and a plausible but wrong pressure indication are separate failure conditions: the latter may delay crew response. If no crew procedure or alert timing is provided, record the response assumption as unresolved rather than classify the effect conclusively. ## Acceptance checks - Covers omission, commission, and misleading behavior across relevant phases. - Each classification has a project definition and traceable rationale. - Assurance-level allocation remains a separate approved task. Missing inputs permit a bounded partial assessment with specific evidence requests, not invented values, failure rates, clauses, approvals, or safety decisions. Keep analysis rows in the delivered document or spreadsheet; they do not create new exchange Item Types or Relationship Types. If proposing model edits, retain the original records and route proposals through the project’s authorized validation and review process. A responsible engineer or authority owns formal acceptance.
