Preserve source IDs, wording, configuration, and assumptions in the output; do not modify a connected system or assert approval. ## Inputs and boundary Obtain the mission or operating concept, system boundary, lifecycle phases, preliminary architecture, energy/material inventories, people and environment exposed, known incidents, and the project’s severity terminology. Identify what is absent. A concept-only analysis can expose hypotheses but cannot close hazards or assign an approved risk level. ## Method Walk through nominal use, startup, shutdown, maintenance, transport, emergency operation, and foreseeable misuse. For each hazardous condition, distinguish the source of harm, triggering circumstance, exposure path, and possible consequence. Include interactions across hardware, software, people, interfaces, and external services. Record initiating events and assumptions without presenting them as measured probabilities. Trace existing controls to the condition they interrupt, separating prevention, detection, mitigation, and recovery. Look for single-control dependence, common causes, and control failure. Compare each candidate with the supplied hazard taxonomy and de-duplicate by causal scenario rather than by shared consequence. Identify analyses needed next, such as FHA, FMEA, fault tree, or test. Leave severity and acceptability unassigned where the project criteria or decision authority are unavailable. ## Deliverable Produce a PHA register with hazard ID, operating phase, system boundary, hazardous condition, initiating circumstance, consequence, exposed party, existing controls, proposed control or investigation, source, assumptions, owner, and status. Distinguish observed evidence from analysis hypothesis and identify decision requests explicitly. ## Miniature example A battery heater stuck on during ground servicing could overheat adjacent material. The thermal cutout is a proposed preventive control; without its independence, trip threshold, and test record, mark control effectiveness unverified. A correctly bounded entry can remain open without a numerical likelihood. ## Acceptance checks - Every row describes a hazardous condition and consequence, not a vague component fault. - Controls and evidence refer to the same configured design. - Open questions and human disposition are visible. Missing inputs permit a bounded partial assessment with specific evidence requests, not invented values, failure rates, clauses, approvals, or safety decisions. Keep analysis rows in the delivered document or spreadsheet; they do not create new exchange Item Types or Relationship Types. If proposing model edits, retain the original records and route proposals through the project’s authorized validation and review process. A responsible engineer or authority owns formal acceptance.
