Preserve source IDs, wording, configuration, and assumptions in the output; do not modify a connected system or assert approval. ## Inputs and boundary Require the technical objective and baseline, architecture, known uncertainties, program phase, evidence, and project risk criteria. Use the exchange model’s existing Risk fields only when proposing exchange records; its locked severity and likelihood scales are not a generic safety matrix or numeric calculation. ## Method Form each risk as cause → uncertain event → consequence for a named objective. Separate already occurred issues from future uncertain events, and distinguish engineering hazards from schedule, cost, or integration risk when the decision process differs. Test each scenario against observed evidence and assumptions. Identify triggers or leading indicators, existing controls, control weaknesses, and plausible alternatives. Apply project-defined qualitative scales only if supplied and describe why the selected category fits; otherwise leave assessment unscored. Check cross-discipline interfaces and long-lead suppliers because these often generate risks outside component analyses. Consolidate duplicates by causal event while retaining distinct consequences. Suggest a response choice—reduce, avoid, transfer, accept, or monitor—only as a proposal; acceptance is an authorized human decision. ## Deliverable Provide a risk register proposal with ID, cause, event, consequence, objective affected, evidence/assumption, current control, category if supported, trigger, response idea, owner, and open decision. If mapped to the exchange model, preserve existing Risk Item semantics and the project’s change-control process. ## Miniature example A thermal-vacuum component has passed bench testing but not cold soak. “If cold-soak startup fails, deployment may miss its power window” is a risk; a cold-soak test is a possible response. The bench pass is evidence of one condition, not proof the risk is closed. ## Acceptance checks - Each risk statement has cause, event, and consequence. - Existing issues and hazards are differentiated. - No unsupported score or automatic acceptance appears. Missing inputs permit a bounded partial assessment with specific evidence requests, not invented values, failure rates, clauses, approvals, or safety decisions. Keep analysis rows in the delivered document or spreadsheet; they do not create new exchange Item Types or Relationship Types. If proposing model edits, retain the original records and route proposals through the project’s authorized validation and review process. A responsible engineer or authority owns formal acceptance.
