## Patient Population:
 - Patients 45-75 years of age
## Measure Description:
 - Percentage of adults 45-75 years of age who had appropriate screening for colorectal cancer
## Exclusions:
 - Patients with a diagnosis or past history of total colectomy or colorectal cancer, must be documented in past surgical history.
 - Patients who were in hospice care during the measurement year.
## Intervention:
Order one of the following stool based tests:
 -  High sensitivity gFOBT within last 12 months
 -  IT within last 12 months.
 -  Order sDNA-FIT every 12-36 months.
   
Order one of the following direct visualization tests:
 -  Colonoscopy every 10 years
 -  Flexible Sigmoidoscopy every 5 years
 -  Flexible Sigmoidoscopy every 10 years in combination with FIT every 12 months
   
Order a CT Colonography (Result captured in Data Integration via Imaging Report) every 5 years

## Satisfaction
Imaging reports or Specialist Consult Report must be uploaded via Data Integration and must also be reviewed by the provider to complete the intervention. Data Integration imaging exams and specialty consult reports to use include:
- Imaging
- Colonoscopy
- Consult Report
- Colonoscopy
