# Scoring Visualizer

## Description

Monitor patient progress over time with the Scoring Visualizer.

For any scored questionnaire:
- Open the visualizer from the Social Determinants action button in the patient summary
- View recorded scores over time
- Hover over data points to see exact values on a given date
- Compare multiple metrics in the same view
- Identify trends and progress over time
