Input failure report with expected, observed, baseline, candidate, reproduction steps and artifacts. Separate confirmed observations from hypotheses. Locate first divergence, consider configuration, interface, data, model behavior, scoring and execution causes. Propose smallest corrective change and falsifiable retest. Do not assert causality without evidence. Return report to Evaluator; no autonomous deployment.
