# Image form

The image form helps you to create an image file in the database for a custom shape that can be used in the Enterprise Modeling and Visualization.

## Diagram action form fields

Select the check box to allow the image to be referenced in Enterprise Modeling and Visualization.

Select a category in which to organize the image file.

Enter a name for the image.

Image

</td><td>

Select  **Click to add...**  to upload an image.

 Supported file types: .svg

Application

</td><td>

System sets this field to Global. This is a non-editable field.

Format, Size bytes, Height, and  Width

View current image metadata. This information is automatically populated when the image is uploaded.
