# Add hazards and log sequence of events for a safety incident

Add hazards andlog a sequence of events for a safety incident as part of its investigation.

## Before you begin

Role required: sn\_ohs\_im.manager, sn\_ohs\_im.agent, or sn\_ohs\_im.operations\_manager

## Procedure

1.  Navigate to **All** > **Health and Safety** > **Health and Safety Workspace**.

2.  Select the incident management icon \(\[Omitted image "list-icon-hs.png"\] Alt text: Incident Management icon.\).

3.  In the **Safety Incidents** list, open a safety incident assigned to you in which you want to add the sequence of events.

4.  Select the **Report an incident** tab.

    The incident playbook opens.

5.  In the **Enter incident summary** stage, under the **Add event details** activity, add the following information.

    1.  Location where this incident happened.

    2.  Add hazards identified during this incident.

    3.  Log the sequence of events as you know them right now.

        You can use the available formatting tools to format the text.

6.  Select **Save**.

7.  To mark this activity as complete, select **Mark complete**.

    The next activity or stage becomes available in the playbook.

8.  If necessary, revisit the sequence of events using the edit icon in the section.


## Result

The logged events appear in the Sequence of events section in the **Incident Overview** tab.

## What to do next

Add people involved in the incident such as injured person, witnesses, and collaborators.
