# Create actions or findings for the smart assessment questionnaire

You can create actions or findings for every question in the smart assessment questionnaire. For example, to report a hazard observed while completing the smart assessment questionnaire.

## Before you begin

Role required: sn\_hs\_rm.safety\_inspection\_writer or sn\_hs\_rm.safety\_audit\_writer

**Note:** To perform this task, the role must match or contain the role specified in the **Assessment reader role** field. This field is set during the smart assessment template configuration. For more information, see [Configure the smart assessment template for safety inspections and audits](https://raw.githubusercontent.com/ServiceNow/ServiceNowDocs/australia/markdown/australia/employee-service-management/health-and-safety-risk-management/hs-configure-smart-assessment-template.md).

## About this task

The actions or finding that are created for the inspections and audits through the Mobile Agent app gets synced with the workspace.

## Procedure

1.  Tap the Home icon \(\[Omitted image "home-page-icon.png"\] Alt text: home icon\).

2.  Select **See all** for the **Smart assessment** section.

3.  Tap the smart assessment record.

4.  Tap the More actions \(\[Omitted image "agent-workspace-more-ui-actions-icon.jpg"\] Alt text: more actions\) icon.

5.  Tap **Actions list**

6.  Tap the More actions \(\[Omitted image "agent-workspace-more-ui-actions-icon.jpg"\] Alt text: more actions\) icon on the **Assessment actions list** page.

7.  Tap the **Create action** button.

8.  Complete the **Action input** form.

9.  Tap the **Submit** button.


## Result

The action record appears in the **Assessment actions list** page and is synced with the workspace.
