# Enroll in a health plan

As an employee, you can submit a question about or enroll in a company health plan such as medical, dental, or vision from the Employee Center.

## About this task

Employees can ask questions about company benefits and enroll in a medical, dental, or vision plan by submitting a request through the Employee Center.

Alternatively, a manager can submit an enrollment request by opening an HR case: [Create an HR case](https://raw.githubusercontent.com/ServiceNow/ServiceNowDocs/australia/markdown/australia/employee-service-management/hr-service-delivery/search-hr-case.md).

## Procedure

1.  From the Employee Center menu, navigate to **Human Resources** > **Benefits** > **Health and wellness**.

2.  Click the **Medical Plan Enrollment**, **Dental Plan Enrollment**, or **Vision Plan Enrollment** request.

3.  Complete the form.

    The fields you see depend on how the form is configured and what fields are selected to display.

4.  Click **Submit**.

    The system creates an HR case and assigns it to the configured HR person. View the status of your request from the **My Requests** tab.
