# Ethics complaints intake form in the Anonymous Report Center portal

Employees can use the Ethics Complaints intake form in the Anonymous Report Center \(ARC\) portal to report an ethics violation incident. Different sets of fields appear on the form based on the active record producers.

The following fields appear on the Ethics Complaints form in the ARC portal when the record producer mapped to the Ethics \[sn\_lg\_ops\_ethics\] table is active.

Users who are the subject of the complaint.Enter the names or email IDs of the involved parties, each name separated by a comma.
