# Set up Legacy assessment template

Create your own questionnaire \(assessment\) template instead of using the default template in the Operational Resilience application to suit your business needs.

## Before you begin

Role required: sn\_oper\_res.admin

## About this task

Create your own questionnaire \(assessment\) template and request the responses from the assessor. This task is optional.

## Procedure

1.  Navigate to **All** > **Operational Resilience** > **Administration** > **Assessment Templates**.

2.  To create an assessment template, click **New** as shown in the following example.

3.  On the Assessment Metric Type form, fill in the fields and click **Submit**.

    The new assessment template is listed in the Assessment Metric Types list as shown in the following example.
