## Before you begin
Role required: sn\_oper\_res.admin
## About this task
Creating a customized HTML document template is optional.
## Procedure
1. Navigate to **All** > **Document Templates** > **All Document Templates** and click **New**.
The following question is displayed on the form: **What kind of document template would you like to create?** The available options are as follows:
- HTML Document Template
- PDF Document Template
2. Select **HTML Document Template**.
3. In the **Table** field, select the **Self Attestation \[sn\_oper\_res\_self\_attestation\]** table.
The sample form is shown in the following example.
\[Omitted image "html-document-template.png"\] Alt text: HTML template.
4. On the form, fill in the fields and click **Submit**.
The new template is displayed in the Document Templates list. For descriptions of the field values, see [HTML Template form](https://raw.githubusercontent.com/ServiceNow/ServiceNowDocs/australia/markdown/australia/governance-risk-compliance/html-template-reference-form.md).
## Result
If you have the sn\_oper\_res.manager role, you can use the HTML template to generate a self-attestation report.
