# Create Contract form

On the Create Contract form, fill in the fields.

|Field|Description|
|-----|-----------|
|Contract model|Type of the contract being created such as Service or Purchase Agreement.|
|Vendor|Vendor of the contract.|
|Contract number|Unique number identifying the contract, as assigned by the vendor.|
|Name|Name or short description of the contract.|
|Parent contract|Parent contract or agreement for this contract.|
|Start date|Date on which the contract takes effect.|
|End date|Date on which the contract expires.|
|State|Current state of the contract, for example, **Draft**.|
|Substate|Current sub-state of the contract, for example, **Awaiting review**.|
|Contract administrator|Internal contact for reviewing, interpreting, and signing contracts, for example, Abel Tuter.|
|Approver|Person who is responsible for reviewing and approving the contract.|
|Business owner|Internal contact responsible for the ownership of goods or services.|
|Description|Detailed description of the contract.|
