For a description of the form field values, see the following table.

|Field|Description|
|-----|-----------|
|Name|Name of the regulatory contact.|
|Regulatory agency|Regulatory agency to which you add the regulatory contact.|
|Active|Option to select if you want to make the regulatory contact active.|
|User|User name of the regulatory contact.|
|Email|Email address of the regulatory contact.|
|Phone number|Phone number of the regulatory contact.|
|Address|Contact address of the regulatory contact.|
|Designation|Designation of the regulatory contact.|
|Apply to global jurisdiction|Contact that also applies to a global jurisdiction.|
