An external facing Personal Data Rights \(PDR\) form configuration record holds the form-wide content that the requester sees across regions.

|Field|Description|
|-----|-----------|
|External intake form title|Main title shown at the top of the public form. For example, `Submit a personal data request`.|
|External intake form sub-title|Short description shown below the main title to give the requester context for the form.|
|Guidance - Request information|Help text shown on the page where the requester selects their request type.|
|Guidance - Contact information|Help text shown on the page where the requester provides their contact details.|
|Guidance - Final review|Help text shown on the review page before submission.|
|Privacy statement URL|Default link to your organization's privacy statement.|
|Legal terms URL|Default link to your organization's legal terms.|
|Terms of service URL|Default link to your organization's terms of service.|
