# Contact form in the ServiceNow CRM for Outlook add-in

Field descriptions for creating a contact record from the ServiceNow CRM for Outlook add-in.

|Field|Description|
|-----|-----------|
|First name|First name of the contact.|
|Last name|Last name of the contact.|
|Email|Email address of the contact pre-populated from the From field of the email header.|
|Business phone|Business phone number of the contact.|
|Mobile phone|Mobile phone number of the contact.|
|Account|Account associated with the contact.|
|Title|Job title of the contact.|
|Time zone|Time zone of the contact. This field is set to the system setting by default.|
|Language|Preferred language of the contact. This field is set to the system setting by default.|
|Notification|Option to enable notifications for this contact.|
