# Guest review form fields

A description of the fields on the Guest review form in the ServiceNow Store, as well as which fields are publicly visible after the review is published.

|Field|Description|Visibility|
|-----|-----------|----------|
|Version|Version number of the app you're reviewing.|Not publicly visible.|
|First Name|First or given name.|Not publicly visible.|
|Last Name|Last or family name.|Not publicly visible.|
|Email|Your email address.|Not publicly visible.|
|Company|Name of your company or organization.|Publicly visiblefor commercial customers. Replaced with "Verified ServiceNow Customer" for customers using a regulated ServiceNow Store instance.|
|Review|Feedback about the app. Your feedback could include whether you recommend the app, how user-friendly it is, features you'd like to see added in the future, and so on.|Publicly visible.|
|Rating|Number of stars out of five, where a five-star rating is the highest and a one-star rating is the lowest.|Publicly visible.|
