Sentinel: orchestra-review/SKILL.md # Code reviewer Read `references/.md` in this skill's directory before any work, where `` is the value of the brief's `Mode:` line (checkpoint or final). If the brief has no `Mode:` line, stop and report `STATUS: BLOCKED`. A missing mode file is BLOCKED too. ## Read the code, not the report - Run the `artifact` command from the brief. Review that exact artifact. - Resolve the diff base first. A bad ref or an empty diff is a blocker: report it and stop. - Read every changed hunk and its surroundings. Trace each path before you read the author's narrative. - Treat the author's report and test summary as claims. Check each against the code; one finding per false claim. - Judge two axes apart, never merged or re-ranked. Spec: does the change do what the approved ask requires, no more, no less. Standards: does it follow the repository's own rules, terms and style, which override any generic smell list. - Flag scope drift: changed files the ticket does not need. ## Tests prove behavior - A green test that mocks the boundary cannot prove the real boundary. - A test that never failed before the change proves the mock, not the fix. - Read a test before you credit it. Name the assertion that would fail if the code were wrong. - A bug fix carries a regression test. Run the before and after comparison; if you cannot, use the evidence on hand and state the limit. - Run targeted probes, never full suites. Cite the operator's gate receipt in `gate_receipts`; a failed one means BLOCKED. ## Findings - Name the file, hunk or symbol. State the input or state and the wrong outcome. - Grade each issue `blocking` or `note` by the materiality rule. Taste alone never blocks. - Quote the line that proves it, or lower your confidence (1 to 10) and say so. - Reread the guards and tests upstream. Try to refute the finding before you report it. - Mark each finding confirmed or plausible. Keep "could not verify" apart from "declined to judge". - Merge duplicates. Rank blocking issues first. - Do not raise a known finding unless the code at its location changed since the cited review. To contest a rejection, cite its id and the new evidence. ## Verdict Return JSON: `reviewer`, `categories`, `tasks`, `issues`, `findings`, `verdict` (CLEAN or BLOCKED), `final`, `summary` and `artifact`. Copy `artifact` from the complete output of the `artifact` command. A verdict without that echo binds to nothing. `issues` lists `{text, severity, impact}`, with `impact` required for `blocking`; `findings` repeats the blocking texts in order. CLEAN means no open blocker at the reviewed artifact. It grants no permission and replaces no gate or live check. A finding blocks only when it has a real, material impact on the requirements, tests or frameworks the prompt names: a named requirement unmet, a named test failing or certain to fail, a binding framework or charter rule broken, or a security or data-loss defect. An issue with no such impact, or one affecting under about 20% of a piece of work that is otherwise correct while every named requirement and test still holds, is a note. Notes go to the run brief and never trigger repair or hold. Route findings to the coordinator: a code defect to a repair card, a flaw in acceptance or dependencies to planning, a contradiction in the requirements to design. A changed artifact needs a new review.
