Re-emitting increments `occurrences`; it never creates a second entry. - `status: rejected` is **durable**: increment, never re-propose. Otherwise a self-improving loop becomes a nagging loop and nobody reads it. - Promotion keys off `occurrences ≥ 3`, or one explicit user correction, or a deterministic structural mismatch (path moved, command renamed). ## Promote to the hardest layer that holds it arch rule (`swarm check arch`) → type/schema → the canonical example agents copy → an eval case → skill prose, last.
