QUALIFICATIONS & GUIDELINES

  1. QUALIFICATIONS

  1. ELIGIBILITY
    In order to be eligible to receive MAP donated medicines and medical supplies, an applicant must provide the following materials:

  1. Fully completed MAP International approval forms, as applicable:

  1. Recipient Application All prospective recipients must complete.

  2. Practitioner's Agreement of Responsibility Inquirers requesting prescription medicines and medical supplies

  3. Project Profile Large volume shipments.

  1. Additional Information:

  1. Recent Annual Report.

  2. Descriptive brochure or any other promotional literature about your organization or project to assist MAP in learning more about the work which will be done.

  3. For U.S. Based Organizations:
    - a copy of your 501(C)(3) tax exempt letter issued by the Internal Revenue Service indicating your status as a charitable or non-profit organization.

  4. For Non-U.S. Based Organizations:
    - an official government notice of the existence of your organization of non-profit purposes.

 

  1. MEDICAL ASSISTANCE PROGRAMS
    The following defines medical supply services available:

  1. CUSTOM ORDERS
    Vital supplies are provided to hospitals in the developing world, in accordance with orders placed by their physicians based on MAP's product listings of available items.

  2. PRE-PACK SERVICES
    Pre-packaged assortment of essential medicines and medical supplies ideal for short-term mission teams, doctors, nurses and/or church mission groups (e.g. Physician Travel Pack).

  3. EMERGENCY RELIEF
    Working through local agencies in countries suffering catastrophic events (i.e. drought, famine, earthquakes, hurricanes, and civil strife), MAP's disaster assessment team ensures medicines and supplies critical to restoring health are securely and effectively distributed among those in greatest need (e.g. Emergency Health Kits).

  4. LARGE VOLUME ORDERS
    This program is for partner groups or agencies that have need of large volumes of produce.t Shipments are primarily ocean container size, but can include large air shipments of truckloads as well. Completion of a Project Profile, obtained by request, is required for each large volume shipment.

  1. Limited Stock: Since MAP's inventory is primarily dependent on donations from the U.S. pharmaceutical industry, MAP has limited control over the types and quantities of medicines and medical supplies which are received. All orders are handled on a "first-come, first-served" basis. Any item not available when an order is received will be canceled from the order. MAP does not back-order any item. However, as inventory becomes available, the item may be re-ordered at a later date

  2. Product Use: All recipients of MAP donated product assumes full responsibility that all medicines and medical supplies will be used outside of the United States. In compliance with the Food, Drug and Cosmetic Act, as amended, and Internal Revenue Service regulations, all medicines and medical supplies cannot be sold or exchanged for property or services and must be used only in treating the ill, needy and infants.

  3. Customs: Documents are carefully prepared to assist you in the customs process. MAP International is not responsible for customs charges, fines or taxes. Should product be confiscated by customs officials and not released for its intended use, MAP International should be notified as soon as possible.

  4. Security: Donations from pharmaceutical and medical supply companies are dependent upon the product being transported and dispensed in a secure manner upon receipt. When making arrangements in other countries, please exercise caution. Shipments to locations in the U.S. for international transport should be opened only AFTER their arrival to the overseas destination. If ANY medicines or medical supplies are lost, misplaced or stolen prior to arriving to their ultimate destination, you are responsible for contacting MAP International immediately.

 

 

  1. ORDERING INFORMATION

CUSTOM/LARGE VOLUME ORDERS PRE-PACK SERVICES
HANDLING CHARGE The Handling Charge must be paid in U.S. dollars. Shipping costs are additional and will be billed to you once they are known.
The Handling Charge assists with one-third of the costs of MAP's computerized inventory system, highly trained staff, extensive storage space and distribution tracking system.
ORDER PROCESSING A minimum of six weeks should be allowed to handle all of the details involved in preparing a large volume order for shipment. Each Pack is pre-packaged and processed within a 1 to 2 day turn around
SHIPPING Recipients are responsible to research and determine the best method for shipping supplies into their area and provide MAP with complete shipping instructions.

MAP staff will work with appropriate freight forwarders to provide quality service and transport of goods.
Shipped U.P.S. ground transport unless expedited delivery is indicated

Shipping charges included in the handling Charge. Expedited shipping subject to an extra charge.

OCEAN FREIGHT REIMBURSEMENT Certain countries qualify for reimbursement of ocean freight expenses which is made available to MAP by USAID.
