- Surgeons can now use their scalpel to damage specific parts of patients brains to affect performance in them. This includes areas that affect limb usage, headbutting airlocks, speaking gibberish, and being able to use machinery. All other forms of things that caused brain damage still affect all four as normal.
- Cult runes, staffs of change/polymorph/necromancer, chameleon projectors, explosive implants, vocal implants, wheelchairs, syndie toolboxes, living long balloons, taperolls and various reagents can now be arcane tampered.
- Arcane tampered drinking glasses now refill over time with the player's blood, have a 75% teleport to a random nearby table on their mob's death redisguising as another kind of drink. They also no longer affect mobs instantly, instead taking 2-5 seconds.

- Reworked the way impaired vision gets rendered. The "grate" effect has been removed and instead a sphere of darkness and blurriness smoothly closes-in around your character. At low impairment values (1-2), you will only see a bit of blurriness in the corners of your view distance. At max impairment (10), it looks just like when you were blind before.
- Impairment increases based on your eye damage (with default eyes you start seeing effects from 5 damage, but it now takes 35 damage to go fully blind), whether you have the nearsightedness disability (3), whether you are wearing hats or glasses that reduce your vision (welder hat/goggles for instance, 5), and decreases when wearing prescription glasses (-3) or using advanced eyes (-2). It can also increase temporarily from stuff that could previously temporarily blind you. Let it be known that non-nearsighted people can still wear prescription glasses to counteract other sources of vision impairment, such as welder helmets.
- Imidazoline has been greatly enhanced at stopping temporary blindness over a shorter period of time, on top of its previous benefits.
- Reworked the way blurry vision gets rendered. It now makes your whole screen (minus the UI) slightly blurry instead of a white fog appearing over everything (aka cumvision).
- Fixed being able to see beyond the range of the old blindness sprites when using binoculars, sniper, or farsight gene.
- Holding a cane now lets you see on the tile adjacent to yours regardless of what type of blindness you have.
- Prescription Glasses now have the reverse effect for players who are NOT nearsighted. Note that this applies specifically to glasses and scanner that have prescription specified in their name. CMO Medhuds and any other types of glasses that also provided nearsightedness correction still correct the eyesight of nearsighted players, without making it blurry for non-nearsighted ones.
- The Cheap Glasses cargo crate now also contains a pair of cosmetic glasses. Those look just like regular prescription glasses but with none of the eyesight correction, allowing non-nearsighted player to wear those without seeing things blurry.
