First time I experienced it was almost 3 months ago when I was
level 27 or so (I'm level 40 now). It's more prevelant when
you're using the song on mobs closer to your level; which
incidently is also when the Pixie Strike bug most often
happens. It's annoying, but in my experience doesn't happen
often enough to be a major hassle unless several of them got
stringed together.
