It's a bug. I've seen it happen many times, and haven't been
able to figure out a pattern. The most dramatic was when I
went LD in Hate and dropped from my group. I finally got back
in about 10 minutes later, everyone was fighting an
almost-dead karikuei (or whatever). I equipped my weapons,
hit it maybe a dozen times, and got the kill.

I think some spell, probably a DoT, is somehow resetting or
rolling over the damage list. The same bug was also probably
the culprit in several mobs disappearing when they died while
we were in Fear.
