Right side of the plaque is preserved. Only the front is smooth. Broad upper and lower margins

The S-shaped sign abbreviates καί in line 1. A dash divides words in line 1. Omega is made with two curving strokes that do not touch

A cross cut into the last sigma at the end of line 3

In 1946 it was at Caesarea (IAA Archives, 7 Aug. 1946, "Caesarea Antiquities Room," no. 105, photo 36.198), but it is no longer there.
