After 5 failures: Log to dead-letter queue, move to next task. ``` **Why PRE-ACT ATTENTION matters** (from planning-with-files pattern): - Context drift is silent - agents don't notice they've drifted off-task - Forcing goal re-read before each action catches drift early - Prevents "correct solution to wrong problem" failure mode - Cost: One file read per action.
