(yes/no) - If no: what is the gap, and what is the likely cause? ## Step 5: Improvement plan For each criterion below target: 1. Root cause: why are patients not meeting this standard? (knowledge gap, system gap, process gap, resource gap) 2. Specific action to address the root cause 3. Owner and deadline 4.
