Must ask user. Examples: ambiguous scope, conflicting requirements, missing access/permissions, unknown target behavior - **MINOR** - Can fix in plan without asking. Examples: missing test file paths, unclear variable names, minor ordering preferences - **AMBIGUOUS** - Apply reasonable default, document the assumption.
