The top long-task failure mode is compounding silent errors. The model taps a button, assumes it worked, proceeds to the next step. But the tap missed. Or hit the wrong thing. Or nothing happened. Ten steps later the task is hopelessly off-track and the model doesn't know when it went wrong.

The conservative threshold matters. A wrong ✓ is worse than no assertion at all — it gives the model false confidence. So the fallback only confirms when there's strong textual evidence. The ✗ feedback always includes "adapt, don't assume." This is steering: the model's natural tendency after a ✗ is to retry the same action. The feedback pushes it toward adapting its approach.
This is the agent checking its own work. Not the vehicle verifying for it — the agent CHOOSING to verify, using a deterministic truth oracle that sees what the model's 640px JPEG can't resolve.
