THE LOOP-BREAKER ESCALATION LADDER When a screen hits LOOP_LIMIT (6 visits), the orchestrator doesn't immediately grab the wheel. It runs a three-tier escalation: TIER 1 — NUDGE (first time this screen hits the limit). `loopNudged.add(sig)` gates this to once per screen per task. The agent gets a gate note naming the loop and listing what's already been tried here. It chooses its own escape. The visit counter is backed off by 2 so the nudge has room to work before Tier 2 fires. TIER 2 — MOTOR RECOVERY (if the nudge didn't work). Escalates within itself: `tryAdvance()` first (tap a popup/continue button, staying in-app); then `back` (dismiss a dialog/menu without leaving the app); then `home` as last resort (which risks drift away from the target). TIER 3 — GIVE UP (after MAX_LOOP_RECOVERIES = 4 motor recoveries). One-shot tasks stop. Continuous tasks NEVER give up here — they reset `loopRecoveries` and cycle back to Tier 2 (BACK-first again, not HOME-spam).
