When the task says "create a new note" and "draw," the orchestrator ensures a blank canvas exists before the model touches anything. Without this, the agent draws on whatever note was last open — the owner's "it drew the cat on top of the old totem" bug.

Three detection paths:
1. If `create_note_btn` is on screen (the notes list), tap it. Fresh note created, `freshNoteEnsured = true`.
2. If we're already in a pen editor BUT `hw_toolbar_undo` is NOT disabled (meaning there's existing ink), press the back/navigate button to return to the list and start fresh.
3. If we're in a pen editor AND undo IS disabled, the canvas is already blank — mark ensured, no action needed.

This is state preparation, not decision-making. The model decides WHAT to draw. The deterministic code ensures there's a clean canvas to draw ON. The distinction maps exactly to the vehicle/driver analogy: the vehicle ensures the road is clear; the driver decides where to go.

The `freshNoteEnsured` flag prevents re-triggering. Once a fresh note exists (by any path), the guard stops checking. One blank canvas per task, not one per step. And it only fires when the objective explicitly says "new note" or "create note" — a task that says "draw in the current note" would never trigger it.
