After REORIENT_AFTER (3) "lost" events — loop recoveries, drift corrections, oscillation detections — the agent's current plan is clearly not working. The stale plan is telling the agent to go somewhere it can't reach or do something it can't do from here. The reorient mechanism throws out the plan and replans FROM THE ACTUAL SCREEN.

This is distinct from every other recovery mechanism. The loop breaker handles a single stuck screen. The drift guard handles being in the wrong app. The hang watchdog handles a wedged inference. The reorient handles a higher-level failure: the plan itself is wrong. The agent followed a valid plan to a screen the plan didn't anticipate, and no amount of single-step recovery will get it back on track.

The replan context carries what just happened and what's failed, so the new plan can avoid repeating the dead end. The planner's prompt includes "WHAT'S HAPPENED SO FAR (the earlier plan got stuck — take a DIFFERENT route)" with the current screen and failed actions. This is the FSD equivalent of recalculating the route after three wrong turns — don't just recalculate from the original destination, recalculate from WHERE YOU ARE NOW with knowledge of which roads are blocked.

Bounded at MAX_REORIENTS (3). Three fresh plans. If all three fail, the task stops. This prevents infinite replanning — if three independently generated plans all fail to reach the goal, the goal is likely unreachable from the current state, and continuing would just burn battery.
